1. Poor initial Requirements capture & documentation
2. Poor planning & manpower allocation
3. Initial overselling of the solution by the Pre-Sales guys
4. Customers who rush development un realistically
5. Poor testing & quality gateway
6. Uncontrolled Requirements & increase of cost (uncontrolled requirements escalates cost)
7. Mis-communication OR No-communication among the team members OR between the development team and the customer’s team
8. Poor handling of “Change Management” at customer’s side (The actual users were not made aware of whats going to change when the software is implemented OR how their daily work will be different than what was before – e.g. Manual to Automated)
